Job Summary
Evaluate and develop individualized treatment plans for patients in skilled nursing, assisted living, and independent living environments. Provide skilled physical therapy services in accordance with physician orders while collaborating with interdisciplinary team members. Educate patients and caregivers to support recovery and independence, and maintain accurate and timely documentation. This role offers a full-time or PRN position with a $5,000 sign-on bonus for full-time staff at Trinity Rehab in Salisbury, NC.
