PT - Physical Therapist

Provide skilled physical therapy evaluations and treatments in accordance with physician orders. Develop and update individualized care plans and therapy goals while maintaining timely and accurate documentation. Collaborate with interdisciplinary clinical teams regarding resident progress and needs, actively participating in care planning meetings. Assist with education and training initiatives as needed to support a safe, respectful, resident-centered care environment. Work with diverse populations including short-term rehabilitation residents, orthopedic recovery, neurological conditions, and long-term care maintenance therapy needs.
