Job Summary
Provide skilled physical therapy evaluations and treatments in accordance with physician orders. Develop and update individualized care plans and therapy goals while maintaining timely and accurate documentation. Collaborate with interdisciplinary clinical teams regarding resident progress and needs, participating in care planning meetings and assisting with education initiatives. Support a safe, respectful, resident-centered care environment for short-term rehabilitation, orthopedic recovery, neurological conditions, and long-term care populations. Requires a state license, two years' experience preferred, and regular interaction with residents and staff.
Required Qualifications
- Graduation from an approved school of physical therapy
- State license to practice physical therapy
- Ability to direct others
- Intermediate or more advanced level English language proficiency (reading, writing, speaking, understanding)
