Job Summary
Provide home-based therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while assisting patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients and families on their plan of care, established goals, and safely assisting with exercises as part of a home health team focused on patient outcomes. Requires a Virginia PT license, CPR certification, and knowledge of home health regulations.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PT license in Virginia
- Current CPR certification
- Knowledge of state and federal home health regulations
- Valid Driver's License
- Reliable transportation
Desired Qualifications
- Maximum of 2 years of Physical Therapy experience
- Ideally in home healthcare
