PT / Physical Therapist

Provide home-based therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while assisting patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients and families on their plan of care, established goals, exercises, and self-care techniques. Work as part of a home health team with 1:1 therapist-to-patient ratios to help patients regain strength and reach their goals in a customized home setting. Requires a Virginia PT license, CPR certification, and valid driver's license.
