Job Summary
Evaluate patients' functional status and mobility, then develop individualized care plans to improve strength, balance, and independence. Provide hands-on therapy at the patient's home while educating caregivers on safety strategies and treatment exercises. Monitor progress to adjust plans, support discharge planning, and collaborate with nurses, physicians, and PTAs for coordinated care. Document all care accurately within the EMR and supervise PTAs per state guidelines. Requires a current DPT or Master's degree, state license, 1–2 years of experience, and valid driver's license. Flexible scheduling and autonomy in a field-based environment.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
