Job Summary
Perform physical therapy evaluations and treatments based on physician orders, including adaptive equipment procurement. Establish measurable goals and treatment plans to maximize patient independence and function. Maintain communication with the treatment team, physicians, patients, and families while ensuring thorough documentation for billing. Supervise Physical Therapist Assistants and aides in direct patient care activities in accordance with state practice acts. Provide 1:1 care to patients and families in the Cookeville, TN home health setting.
Required Qualifications
- Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
- At least (2) years of appropriate experience as a physical therapist
- Valid license form the state of practice
- Possess and maintains current CPR certification
