Job Summary
Make home visits to clients in designated geographic territories, performing diagnostic tests and measurements for mobility, strength, balance, and safety assessments. Implement integumentary protective interventions to prevent and repair wounds, then develop and revise individualized care plans including manual therapeutic exercises and gait training. Continually assess client progress, participate in multidisciplinary care planning with nursing, and educate clients and family members on rehabilitative care and independent living skills. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current VA Physical Therapist license, graduation from an accredited program, and strong computer skills with EMR experience. Base pay ranges from $67.00 to $72.00 per point.
Required Qualifications
- A current VA Physical Therapist - PT license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
