Job Summary
Evaluate, develop, and implement individualized treatment plans based on physician referrals for patients in outpatient and skilled nursing facility settings. Provide skilled physical therapy services, monitor progress to adjust plans for optimal outcomes, and maintain accurate documentation in compliance with regulatory standards. Collaborate with interdisciplinary teams, supervise Physical Therapist Assistants and therapy aides, and educate patients and families on treatment goals and home exercise programs. Requires a current North Carolina PT license and graduate of an accredited program; offers flexible part-time and PRN scheduling with a supportive, team-focused environment.
Required Qualifications
- Current North Carolina Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program
- Strong clinical skills and ability to work independently across multiple settings
- Excellent communication and organizational skills
