Job Summary
Physical Therapist (PT) to join Summit Home Care and Hospice in Lancaster, OH. Responsible for providing physical therapy services to patients in home settings under a physician's plan of care, assessing therapeutic status, developing and directing PT programs, evaluating home environments, and guiding patients and caregivers in the use of therapeutic devices. Will supervise PTAs, participate in discharge planning, prepare clinical progress summaries, and collaborate with the care team. May train patients in prosthetic device use, identify needs for additional home health services, and provide in-service education. Requires active Ohio PT license, degree in physical therapy (BS/MS/PhD), CPR certification, TB testing, valid driver’s license, and prior home health/rehabilitation experience; travel to patient homes and local office training; full-time schedule with on-the-road patient visits and remote components.
Required Qualifications
- Active physical therapy license in the state of Ohio
- Bachelors, master's or doctorate in physical therapy from an Accredited Program, Required
- Possess and maintain a current CPR certification
- Recent TB Test
- Valid driver's license and reliable transportation with active automobile insurance
- Two years of appropriate experience as a physical therapist preferred
- Community/home health experience is preferred
