Job Summary
PT Home Health SOC visits across Los Angeles and Ventura County. Assess therapeutic and rehabilitative status, develop and update treatment plans, ensure high-quality patient care, and utilize AI-driven OASIS documentation to reduce charting time. SOC visit pay ranges up to $145 per visit; flexible hours and coverage areas; training and ongoing support; licensure in California required; experience in home health preferred. Include DPT degree, CA PT license, strong communication, and ability to perform Start of Care assessments; AI-assisted documentation enables focus on patient care.
Required Qualifications
- Doctorate of Physical Therapy (DPT) from an accredited program
- Current and unrestricted CA PT license; or ability to apply
- Degree from an accredited DPT program
- Excellent Communication Skills
- Home Health experience preferred; not required
