Job Summary
PT SOC (Start of Care) visits across Los Angeles and Ventura County for Home Health Links. Responsibilities include assessing patients’ therapeutic status, developing and updating treatment plans, ensuring high-quality patient care using clinical reasoning, and maintaining accurate records. AI-assisted OASIS documentation is used to streamline charting time, enabling more focus on patient care. Role offers flexible scheduling, nearby patient locations, fast electronic documentation, training and ongoing support. Requires a Doctorate of Physical Therapy and active California license (or ability to apply); Home Health experience is preferred but not required.
Required Qualifications
- DPT degree from an accredited program
- Current and unrestricted California PT license or ability to apply
- Excellent communication skills
