Job Summary
Provide support and assistance to individuals with disabilities in their daily activities; develop and implement individualized care plans; administer medications and monitor health conditions as needed; provide transportation to appointments and community activities; assist with meal planning and daily living tasks; maintain accurate records of client progress; utilize CPR and First Aid certification and experience with behavior management techniques and electronic health records; require reading English for medical prescriptions and directions; must pass background checks and a TB test; part-time shifts include 7AM-3PM and 3PM-11PM in Philadelphia, PA.
Required Qualifications
- High School Diploma or equivalent
- Valid Driver’s License for more than 1 year
- Qualified to work in the US
- Certification in CPR and First Aid
- Experience working with individuals with disabilities
- Experience with electronic health records
- Ability to pass a criminal background check and drug screening
- Complete physical examination, including tuberculosis test
- Ability to read and understand English as it relates to reading medical prescriptions, directions, policies, procedures and directions for managing the daily life and activities of Individuals and a safe and healthy home
