Job Summary
Provide recovery-oriented rehabilitation services in group or individual settings for adults in skilled nursing and assisted living facilities. Join a multidisciplinary team and deliver clinical care as a Licensed Clinical Social Worker (LCSW), LISW, LPCC, LCMHC, or LMFT (or Psychologist with doctoral credential) to address anxiety, depression, and other mental health needs. Responsibilities include assessing residents with consent, developing and following treatment plans, performing written/verbal consultations with primary care physicians and facility staff, documenting treatment plans and sessions within 48 hours, communicating with facility staff, and adhering to HIPAA rules and regulatory standards. Requires appropriate state licensure and ongoing professional practice; competencies include psychosocial assessment, psychotherapy techniques, DSM knowledge, data-driven decision making, and strong written/verbal communication. Required Qualifications
- Licensed Clinical Social Worker (LCSW) or equivalent (LISW, LPCC, LCMHC, LMFT) with Master’s degree and current state license
- Doctoral-level Psychologist with Doctoral degree and current license (if practicing as Psychologist)
- Ability to develop and follow treatment plans; document treatment plans and sessions; maintain patient confidentiality per HIPAA; operate within regulatory guidelines; collaborate with facility staff and clinical supervisor
