Psychotherapist (LCSW/LPC/LCMHC)

Provide recovery-oriented rehabilitation services to adults residing in skilled nursing and assisted living facilities, delivering psychotherapy in group or individual settings as part of a multidisciplinary team. Responsibilities include developing and following individualized treatment plans, conducting written/verbal consultations with primary care providers and facility staff, documenting treatment plans and psychotherapy sessions within 48 hours for medical records, maintaining open communication with facility staff, adhering to HIPAA and regulatory requirements, and applying evidence-based assessment and psychotherapy techniques. Licensure requirements vary by discipline (LCSW/LISW/LPCC/LCMHC/LMFT or doctoral-level psychologist) and must be current and licensed in the practicing state. Knowledge areas include assessment techniques, DSM tools, regulatory rules for psychotherapy, and confidentiality. Skills include data gathering and analysis, evaluating patient problems, clear written and verbal communication, initiative, discretion, teamwork, and handling confidential information.
