Job Summary
Provide recovery-oriented rehabilitation services as a Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) in group or individual psychotherapy for adults residing in skilled nursing and assisted living facilities. Develop and follow treatment plans to meet residents’ needs, conduct written/verbal consultations with primary care physicians, family, facility staff, and Eventus WholeHealth teams, document treatment plans and psychotherapy sessions within 48 hours, maintain open communication with facility staff, adhere to HIPAA and regulatory guidelines, and collaborate with clinical supervisors when facing unfamiliar requests. Maintain professional conduct and confidentiality while delivering care in a multidisciplinary setting.
Required Qualifications
- Master’s degree in social work
- current license to practice in the state of practice (LCSW or LISW)
- licensure as social worker in the state where you will practice
- Knowledge of DSM and psychotherapy techniques
- Ability to maintain patient confidentiality and HIPAA compliance
- Ability to work within long-term care facilities (skilled nursing or assisted living) and collaborate with multidisciplinary teams
