Job Summary
Provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities. Join a multidisciplinary team of clinicians to support adults with anxiety, depression, and other mental health needs; develop and follow treatment plans to meet each resident’s needs; document psychotherapy sessions and plan updates; communicate with facility staff and primary care providers; maintain patient confidentiality and HIPAA compliance; utilize DSM guidance and evidence-based interventions; demonstrate initiative, judgment, and collaboration to improve quality of care.
Required Qualifications
- Licensed Clinical Social Worker (LCSW)
- Licensed Independent Social Worker (LISW)
- Licensed Professional Clinical Counselor (LPCC)
- Licensed Clinical Mental Health Counselor (LCMHC)
- Licensed Marriage and Family Therapist (LMFT)
- Master’s degree in social work
- current license to practice in the state
