Job Summary
Provide recovery-oriented psychotherapy services in long-term care settings (skilled nursing and assisted living) as part of a multidisciplinary team; assess residents with consent to determine individual needs; develop and follow treatment plans to meet each resident’s needs and ensure services are medically necessary; perform written or verbal consultation with residents' primary care physician, family, facility staff, and Eventus WholeHealth support staff as needed; document treatment plans, psychotherapy sessions, and updates within 48 hours for inclusion in the patient’s facility medical record; meet with Resident Care Coordinator or designated staff to review care needs; maintain open communications with facility staff; adhere to Eventus WholeHealth standards, HIPAA rules, and regulatory guidelines; conduct oneself professionally; consult with clinical supervisor for unfamiliar requests; possess knowledge of DSM and assessment/psychotherapy techniques; skills in data gathering, evaluation, and communication; ability to work collaboratively with the health care team; ability to react calmly in emergencies; ability to make decisions and contribute to continuous quality improvement.
Required Qualifications
- Therapists- Licensed Clinical Social Worker (LCSW), Licensed Independent Social Worker (LISW), Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Mental Health Counselor, (LCMHC) or Licensed Marriage and Family Therapist (LMFT) - Master’s degree in social work, and a current license to practice in the state in which you will practice, required.
