Job Summary
Develops, implements, and evaluates patient care plans while managing clients in crisis using therapeutic skills and assessment techniques. Writes treatment and discharge plans based on individual diagnoses, ages, and clinical presentations, ensuring accurate documentation in medical records. Collaborates with guardians and community agencies to coordinate service delivery and complete timely billing information. Designs and implements effective patient education programs while maintaining open communication with the healthcare team. Works in patient care areas including offices and conference rooms, requiring frequent movement and coordination with staff across various disciplines.
Required Qualifications
- Master's degree in a relevant human services field such as Social Work, Counseling, Psychology, or Marriage and Family Therapy from an accredited school of graduate education
- Full licensure in applicable state
- 2 years clinical experience
- CPI and CPR training
- Adult experience highly preferred
