Job Summary
Provide direct, supportive care and therapeutic interventions as part of the BH/ID Specialized Team in home and community settings. Responsibilities include maintaining a therapeutic relationship, modeling appropriate boundaries, ensuring a safe and neat living environment, teaching and assisting with skills per each service plan, accompanying residents to appointments or activities, facilitating social engagement, and coordinating with families. Requires collaboration to promote independence and functioning of service participants, crisis management and adherence to safety protocols (including CPR/First Aid). Active driver’s license and a Bachelor's degree are required. This role offers opportunities for professional development and advancement within PATH.
Required Qualifications
- Bachelor's degree required
- 2 years of mental health experience working with individuals in mental health and/or co-occurring settings
- 2 years of experience in human services, paid or unpaid, preferably with adults with psychiatric and/or intellectual disabilities
- Active Driver's license required
