Job Summary
Collaborate with the Well-being team and multidisciplinary staff to evaluate resident admissions and facilitate their integration. Conduct individual follow-ups through scheduled visits and interviews, while co-authoring personalized care plans and documenting session syntheses in the Medicor software. Provide psychological support to staff regarding difficult cases and assist families in caring for their parents. Actively contribute to facility meetings and develop specific care strategies for various pathologies.
