Job Summary
Provide in-home mental health crisis intervention and outreach to clients in the community, delivering psychosocial evaluations, short-term service planning, and ongoing care coordination as part of a mobile crisis team. Collaborate with an interdisciplinary team to assess mental health service needs, respond to dispatch calls within two hours, and serve as a liaison to community agencies to ensure uninterrupted care from counseling to long-term referrals. Perform thorough case documentation, encourage engagement with services, and potentially provide clinical supervision for Mental Health Technician, graduate Social Worker students, or junior staff. Requires an MSW and NYS licensure, with at least one year of healthcare social work experience; bilingual abilities may be required.
Required Qualifications
- Current registration to practice as a Licensed Master Social Worker in New York State
- Master's Degree In Social Work from a graduate school accredited by the Council on Social Work Education
- Minimum of one year experience as a Social Worker in a health care setting
- Valid driver’s license may be required
- Bilingual skills may be required
