Job Summary
Coordinate services, programs, and resident performance oversight for the adult mentally ill population, including care planning, discharge planning, and facilitating skills or psycho-social groups. Manage data collection and review while handling confidential information with professional discretion within an interdisciplinary team environment. Engage families in the process and utilize assessment, advocacy, and communication skills to support resident independence and prevent re-hospitalization. Requires a bachelor's degree in psychology, social work, or related human services, with one year of experience working with adults mentally ill preferred. Position is 5 days a week, 8 hours per day.
Required Qualifications
- Bachelor's degree in psychology, social work, or related social services / human Services field such as sociology or special education
- Must be able to work in an interdisciplinary team environment
- Must be able to handle confidential data with professional discretion
- Position is 5 days/weeks 8 hours per day
Desired Qualifications
- One year working with the adult mentally ill population; preferred but not required
- Prefer experience with caseloads, crisis intervention, documentation, care planning with involving families in the process, knowledge of the MDS, discharge planning, facilitating skills groups and psycho-social groups and problem solving
- Prefer knowledge of MDS 3.0 along with basic computer skills
- Charting experience preferred
