Job Summary
Conduct diagnostic evaluations, manage psychiatric medication regimens including long-acting injectables and MAT, and deliver harm-reduction-oriented care to unsheltered individuals with serious mental illness and substance use disorders. Utilize flexible modalities such as field visits, telehealth, and engagement encounters to improve participant access. Coordinate care transitions, consult with external providers, and lead clinical team meetings to develop evidence-based treatment plans and staff education. Maintain treatment schedules, document all encounters, and adhere to NYS OMH SOS program guidelines. Work part-time, 8 hours weekly from home with travel across Chemung, Steuben, Onondaga, and Oswego counties.
Required Qualifications
- Active, unrestricted NYS license as a Nurse Practitioner (NP), Medical Doctor (MD), or Doctor of Osteopathic Medicine (DO)
- Board certification or board eligibility in Psychiatry (or Psychiatric-Mental Health Nurse Practitioner certification)
- Valid DEA registration with ability to prescribe controlled substances in NYS
- Experience providing psychiatric care to individuals with serious mental illness, substance use disorders, and/or co-occurring conditions
- Familiarity with PSYCKES and electronic medical record (EMR) documentation
- Ability to work in community-based, field, and telehealth settings
- Punctuality and reliable attendance
- Knowledge of homeless resources, shelter systems and transportation systems
- Knowledge of counseling principles and methods for mental illness and substance use disorders
- Knowledge of treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff
- Demonstrates ability to respect individual/family diversity and maintain confidentiality
- Demonstrates ability to work as a team member
- Knowledge of and ability to work collaboratively with providers and county/community health and human services
- Ability to demonstrate excellent communication skills both oral and written as well as strong interpersonal skills
- Proven ability to work independently and to manage time appropriately
- Strong organizational skills
- Computer literate
- Must be able to pass computer documentation competency testing for all software platforms used within the program
- Candidates will need a NYS driver's license
- Must own or have access to reliable transportation that enables them to fulfill travel requirements of the job including but not limited to, daily visits to members' homes
- Candidate must be willing to travel throughout Chemung, Steuben, Onondaga & Oswego Counties on a regular basis
- Grade 313: This is a part time position, 8 hours per week, working from home, but does require travel throughout Chemung, Steuben, Onondaga & Oswego Counties
Desired Qualifications
- Experience with street medicine, assertive community treatment (ACT), or other outreach-based psychiatric care models
- Experience administering long-acting injectable antipsychotics and/or Medication Assisted Treatment (MAT), including buprenorphine and extended-release naltrexone
- Familiarity with clozapine therapy protocols and monitoring requirements
- Experience working with individuals experiencing homelessness, housing instability, or involvement in the criminal justice system
- Training in trauma-informed care, motivational interviewing, and/or de-escalation techniques
- Bilingual or multilingual abilities a plus
