Job Summary
Provide comprehensive psychiatric services to youth and families enrolled in Youth ACT and adults in Adult ACT programs. Prescribe medications, monitor drug regimens, and conduct on-site, community-based, or home visits for enrolled participants. Offer clinical expertise and consultation to the ACT Team, collaborate with county and community partners, and participate in quality assurance, audits, and utilization reviews. Develop and review service plans while engaging in formal and informal staff training. This full-time role requires 40 hours per week, community travel, and a New York license with one year of experience working with mental illness.
Required Qualifications
- Must have a minimum of one year experience working with youth and adults diagnosed with mental illness
- Must be fully licensed Nurse Practitioner in the State of New York
- CPR Certification
- Must be able to work independently
- National Provider Identification (NPI)
- Federal Drug Enforcement Administration (DEA) number
- NYS Bureau of Narcotic Enforcement (NYS DOH BNE) registration
- NYS Medicaid Enrollment
