Job Summary
Licensed LPN to join Harbor’s Toledo team delivering care to clients with mental health/substance use needs. Responsibilities include reviewing and updating treatment plans, medication management (orders, dispensing, and central pharmacy coordination), coordinating insurance forms and pre-certifications, maintaining medication rooms and records, supporting crisis intervention and pre-hospital screening, providing referrals to resources, completing peer reviews and clinical documentation, initiating lab requests, and attending meetings. Requires an Associate’s degree, Ohio LPN license, 2 years psychiatric-nursing experience preferred, proficiency with MS Word, CPR/BLS and NCI/CPI within 90 days, and a valid driver’s license with insurable driving history. Required Qualifications
- Associate’s degree from an accredited school of Nursing
- Current LPN license in Ohio
- Two years’ experience in psychiatric nursing preferred
- CPR/BLS and NCI/CPI certifications within 90 days
- Valid driver’s license and insurable driving record
