Psychiatric Nurse
Job Summary
Provide community-based psychiatric nursing services and coordinate behavioral and physical health care for service participants. Conduct home-based visitations, monitor medications per physician orders, and participate as part of an Assertive Community Treatment (ACT) team to help participants achieve maximum functioning in the community. Complete health and biopsychosocial assessments, coordinate routine medical, vision, lab, and dental appointments, and maintain timely service documentation. Collaborate with community agencies and families to support treatment goals and ensure coordinated care. Maintain medical history and ongoing treatment records. Requires PA RN licensure (Board Certified) and prior experience in community mental health; strong communication, organization, and analytical skills; local travel and on-call emergency coverage; on-site presence with the team.
Required Qualifications
- Board Certified Licensed RN in the State of PA
- Prior experience in servicing community mental health facilities and their psychiatric needs
- Comprehensive knowledge of psychiatric, clinical and psychosocial rehabilitation practices
- Understanding of the Assertive Community Treatment (ACT) model of service
- Must be able to communicate both verbally and in written form with every level of the behavioral health system
- Must have very strong organization and analytical skills
- Must be available for local travel; valid driver’s license with an acceptable driving record required
- May require verbal crisis intervention assistance in emergency situations
- As essential personnel, may be required to report to work during emergencies including inclement weather
