Job Summary
Travel daily to assigned nursing homes and memory care facilities to provide initial evaluations, continuing therapy services, and cognitive testing for patients with dementia and neurocognitive disorders. Document the need and extent of services for each patient while creating positive professional relationships with client facilities, patients, and their families. Work within a multidisciplinary team to promote patient safety, functionality, and dignity, supported by in-house scheduling, coding, and billing departments.
Required Qualifications
- Proof of State licensure in Kentucky
- Pass pre-employment background check and drug screening
- Complete credentialing process
