Job Summary
Conduct comprehensive psychiatric evaluations and mental health assessments for long-term care residents, develop and implement individualized treatment plans, prescribe and manage psychotropic medications in accordance with Medicare and LTC best practices, implement Medicare-compliant gradual dose reduction (GDR) strategies, monitor and treat chronic mental health conditions (e.g., depression, anxiety, bipolar disorder, schizophrenia, dementia-related behavioral disturbances), collaborate with interdisciplinary teams (nursing, medical providers, social services, rehabilitation, facility leadership), and document assessments, treatment plans, and progress notes in the EMR; work closely with facility staff to ensure coordinated psychiatric care, participate in care plan discussions and facility meetings, and maintain compliant documentation under ethical and regulatory guidelines; role focuses on geriatric behavioral health within LTC settings with flexible scheduling and access to ongoing clinical support and continuing education.
Required Qualifications
- Active, unrestricted PMHNP or PMHNP-BC license
- Valid DEA registration
