Job Summary
Psychiatric Care Coordinator (RN) collaborating in behavioral health to coordinate psychiatric care across treatment levels, conduct RN-driven care planning, perform intake and telehealth assessments, and educate patients. Responsibilities include outreach to patients who missed visits, warm handoffs between care levels (e.g., IOP), coordination with external providers (IOP, visiting nurses, specialty services), managing referrals, facilitating release of information, triaging urgent clinical concerns, and supporting lab coordination and pharmacogenetic testing. The ideal candidate supports LGBTQ health care, demonstrates strong communication and time-management skills, and contributes to patient engagement and self-management through education and supportive interventions. Requires High School Diploma and Connecticut RN license; 1 year clinical experience; BLS; and willingness to meet vaccination/exemption requirements.
Required Qualifications
- High School Diploma
- Unencumbered RN License in the State of Connecticut
- At least 1 year of clinical experience
- Experience maintaining a trauma-informed space
- Ability to relate to a broad range of people and communicate clearly, using tact, discretion, and sensitivity with patients
- References required for short-listed candidates
- A criminal background check absent offenses impacting ability to perform role
- Active BLS certification
