Job Summary
Provide individual and group psychosocial rehabilitation interventions for service recipients in an adult residential treatment program. Engage individuals in all phases of the psychiatric rehabilitation process, assisting them in identifying goals for their individual service plans and practicing skills such as attending appointments or participating in leisure and vocational activities. Link residents to community resources and collaborate with physicians, service coordinators, and other healthcare professionals to achieve personal goals. Develop psychiatric rehabilitation goals, complete progress notes, and annual evaluations while consulting with the full team including clinicians and nursing staff. Assist individuals in accessing medical, social, and financial services and follow up on discharge planning. Maintain a driver's license and vehicle for community outings, performing CPR and crisis management interventions once trained.
Required Qualifications
- Experience working with individuals who have a mental illness and/or intellectual or developmental disability
- Driver's License
- Insured vehicle
- Ability to use an Electronic Health Record
- Ability to use electronic timekeeping
