PSC - Patient Services Coordinator Clinical

Coordinate patient visit scheduling across multiple disciplines (skilled nursing, physical therapy, occupational therapy, speech therapy, registered dietitian, social work, home health aide, personal care aide) and manage HCHB tasks and reports. Ensure adherence to SOPs and agency scheduling guidelines; update case managers; reassign visits for staff absences; handle scheduling and reporting tasks, after-hours coverage as needed, and participation on agency committees. Requires strong communication, organization, and computer skills, HIPAA/privacy compliance, and the ability to manage multiple concurrent tasks in an office setting.
