Job Summary
Conduct crisis intervention support by assisting clients with paperwork for admissions, discharges, and transportation to psychiatric placements. Deliver medication, perform welfare checks, and provide phone prompts to clients while coordinating care at crisis stabilization sites. Manage a caseload to establish and implement individual health and wellness plans, guiding clients through coping skills, trauma triggers, and daily living strategies. Assist in pursuing employment and securing integrated, competitive jobs as identified by clients. Maintain all required documentation and report to the supervisor. Work full-time, part-time, or relief shifts Monday through Friday and weekends across the North Country Region with frequent travel. Requires a clean, valid driver's license.
Required Qualifications
- High school diploma or equivalent
- Valid driver's license
- Clean, valid driver's license
- At least 1 year of experience working with individuals with substance use disorders and/or serious mental illness
- Bachelor's degree
- 1 year relevant experience
- At least 3 years of relevant experience working with children involved with mental health, addiction and/or foster care
