Provincial MH&A Flow Specialist

Job Summary

Coordinate the movement of Mental Health patients from rural Manitoba through admission, transfer, and discharge processes by liaising with Service Delivery Organizations and acute care sites. Participate in MH flow calls province-wide and contribute to developing guidelines and educational strategies for patient transitions. Make decisions that optimize facility resource utilization for rural patients while functioning as a facilitator across multidisciplinary teams, physicians, and community resources. Work remotely in a virtual environment, utilizing computerized applications and software to manage these operational flows. Maintain a valid Manitoba driver's license for provincial travel and be prepared for occasional evening or night shifts.

Required Qualifications

  • 5 years' experience working in an acute mental health environment
  • Post-secondary degree in a relevant health profession from an accredited post-secondary education institution
  • Active member in good standing of an associated regulatory body
  • Experience working in a virtual environment
  • Demonstrated staff level leadership capabilities
  • Demonstrated ability to work independently as well as within a team environment
  • Demonstrated ability to function in a pivotal role, as facilitator, in the patient admission/discharge/transfer process, working with patient, family, physician, manager, members of the multidisciplinary team, other programs, community resources, etc.
  • Demonstrated ability to work with all levels of staff (direct care, multidisciplinary team, management)
  • Proficient in concepts related to collaborative practice and inter-professional education and practice
  • Demonstrated ability to assess situations and problem-solve effectively
  • Must have the ability to work effectively and efficiently with computerized clinical applications
  • Must be able to work safely in a typical computerized office environment with frequent meetings, and to lift and carry items of up to approximately 10 kg
  • Must have a valid Manitoba driver's license and use of a personal motor vehicle for regular travel within the province
  • Must be able to work occasional evening or night shifts
  • This term position may end earlier as outlined in your employee handbook
  • Current satisfactory Criminal Records Check (including Vulnerable Sector Search)
  • Child Abuse Registry Check
  • Adult Abuse Registry Check
  • Security check obtained no more than six (6) months prior to the start of employment

Desired Qualifications

  • 3 years of experience in an operational role, particularly with a focus on flow optimization and transition planning
  • 2 years of experience using computerized applications, such as patient flow software
  • Master's degree
  • Working in rural areas
  • Education in Lean Six Sigma or equivalent
