Provider Re-Credentialing

Submit compliance checks including OIG LEIE and SAM exclusion screenings for all active drivers. Provide valid driver's licenses, criminal background checks, sex offender registry screenings, and MVR/DMV records. Complete training verification covering Rules of the Road, safety, emergency procedures, HIPAA, ADA, wheelchair securement, and Fraud, Waste, and Abuse. Submit current vehicle registration and Certificates of Insurance listing all active vehicles by VIN. Upload all documentation to the portal or email compliance@carecar.co if upload issues occur. Failure to complete re-credentialing may result in account review or service disruption.
