Job Summary

Conduct culturally responsive outreach and build relationships with Spanish-speaking individuals to connect them with health, human services, and community resources. Provide translation and interpretation services for in-home visits, office appointments, and program assessments while navigating referrals and care coordination. Deliver education on wellness and available programs, advocate for equitable access, and collaborate with internal staff, healthcare providers, and nonprofit partners to address community needs. Track outreach activities, maintain accurate documentation, and report program integrity concerns. Travel throughout the county to client homes, schools, and healthcare facilities, working regularly scheduled hours including occasional evenings or weekends.

Required Qualifications

  • High school diploma or equivalent
  • Ability to communicate fluently in English and Spanish
  • Ability to accurately interpret and translate information
  • Ability to effectively communicate with individuals with limited English proficiency
  • Ability to establish trusting relationships with diverse populations
  • Ability to communicate effectively with individuals from varied cultural, linguistic, and educational backgrounds
  • Ability to work independently and collaboratively within interdisciplinary teams
  • Ability to maintain confidentiality
  • Ability to organize and prioritize work
  • Ability to utilize computer software and electronic record systems
  • Ability to navigate sensitive or difficult situations with empathy, professionalism, and sound judgment
  • Driver's license valid to drive in the State of Minnesota
  • Bilingual proficiency in English and Spanish
  • Ability to perform the physical and mental demands of the position
  • Ability to attend work during regularly scheduled hours including occasional evening or weekends hours for events
  • Ability to comply with all applicable federal, state, and local laws, regulations, policies, and departmental procedures governing the delivery of human services
  • Ability to provide services in a professional, culturally responsive, impartial, and ethical manner
  • Ability to identify, document, and report suspected fraud, abuse, misuse of benefits, or other program integrity concerns
  • Ability to perform the physical and mental demands of the position
  • Ability to lift up to 20 pounds of force
  • Ability to stand, walk, sit, speak, hear, use hands to finger, handle, or feel, and perform repetitive motions
  • Ability to reach, bend, and operate a motor vehicle

Desired Qualifications

  • Associate degree in Human Services, Public Health, Community Health, Social Work, or a closely related field
  • Experience working with diverse populations and individuals experiencing social, economic, behavioral, or health-related barriers
  • Experience in community outreach, health education, case coordination, resource navigation, or the delivery of human services
  • Knowledge of county service programs
  • Knowledge of community resources
  • Knowledge of health equity
  • Knowledge of social determinants of health
  • Knowledge of community engagement strategies
  • Knowledge of culturally responsive practices
  • Knowledge of principles of trauma-informed care
