Job Summary

Lead planning, coordination, and execution of clinical quality improvement interventions in partner clinics within the Live Healthy Chicago grant program. Manage project timelines, deliverables, and budgets in alignment with grant requirements. Facilitate communication and collaboration among clinical staff, community partners, and program leadership to support intervention goals. Monitor project progress, identify challenges, and implement solutions to maintain momentum and achieve targets. Oversee data collection, quality assurance, and reporting related to clinical intervention outcomes. Support training and capacity-building activities for clinic teams to enhance quality improvement skills. Ensure compliance with regulatory, ethical, and grant-related standards. Prepare regular updates, reports, and presentations for leadership, partners, and funders. Contribute to sustainability planning to embed successful interventions into routine clinical practice.

Required Qualifications

  • Bachelor’s degree in Public Health, Healthcare Administration, Nursing, or related field required; Master’s degree preferred
  • Minimum 3-5 years of project management experience in healthcare, public health, or community-based settings
