Job Summary
Assess patient-generated data via digital technology to manage acute and chronic conditions, providing individualized assessments, education, medication management, and clinical care planning. Rapidly interpret vital signs and symptoms submitted by enrolled patients, addressing needs and goals to improve outcomes while escalating care to providers for adverse health trends. Build long-term relationships with a patient panel through regular communication and motivational techniques to support self-management in the home environment. Work on-site at Waukesha Memorial Hospital with daytime Monday–Friday hours on a 0.5 FTE schedule, utilizing clinical expertise to decrease emergency department visits and hospital readmissions.
Required Qualifications
- Current Registered Nurse (RN) license
- Graduate from an Accredited School of Nursing
- Bachelor's degree in Nursing
- 1-2 years' experience working in hospital and or ambulatory care setting working with patients with chronic diseases
- Previous experience in remote patient monitoring
- Experience in working in an electronic medical record and care management system
- Technological experience as primary healthcare delivery system
- Epic experience
- Strong technological aptitude with the ability to quickly learn and adapt to new software and tools
- Ability to recognize changes in patient condition and make sound clinical decisions independently
- Ability to work on site in Waukesha, WI
Desired Qualifications
- BSN preferred
- 5+ years of RN experience
- Previous inpatient and/or home health nursing experience
- Strong adult medical-surgical, acute care and/or chronic disease management experience
- Excellent assessment, triage and critical-thinking skills
- Strong comfort with technology and learning new digital tools
- Remote patient monitoring or care management experience is a plus
