Job Summary
Coordinate and monitor the implementation of Person-Centered Plans; liaise with the Activities Coordinator to ensure services meet organizational policies, state regulations, and funding requirements; organize scheduling, staffing coordination, and service delivery oversight; monitor participant health, safety, and well-being; serve as primary contact for individuals and families; advocate for rights and goals; assist with accessing community resources, employment opportunities, recreation, and healthcare; support individuals in achieving personal, vocational, and independent living goals; maintain accurate documentation (progress notes, incident reports, assessments, service records); conduct regular audits for compliance; assist with quality assurance reviews; provide guidance and training to Direct Support Professionals; promote best practices in person-centered service delivery; participate in team meetings and interdisciplinary planning; education: Bachelor's preferred in Human Services, Social Work, Psychology, Rehabilitation, Healthcare Administration or related field; experience: minimum two years with individuals with developmental disabilities or related populations; skills: person-centered planning, communication, organization, problem-solving; required credentials: valid driver's license, reliable transportation, background checks, CPR/First Aid or ability to obtain, ability to pass drug and alcohol screenings; travel to residential sites and community locations; driving large passenger vans; Equal Opportunity Employer.
