Job Summary
Program Supervisor responsible for direct oversight of the Care Coordination Team, ensuring Medicaid compliance, managing monthly billing, and using health information technology to inform practice. Supervises and coaches staff in field and office settings; recruits and develops staff; monitors performance through QA, documentation, and adherence to Health Home agreements; ensures 24/7 availability for emergencies; maintains HIPAA-compliant records and multiple EHRs; conducts regular reporting and participates in ongoing professional trainings. Requires LMHC/LMSW/LCSW licensure, 4 years’ experience supervising services for Medicaid recipients with chronic medical and mental health issues, and knowledge of Health Home regulations. Strong leadership, communication, multitasking, and organizational skills are essential.
