Job Summary
Oversee the Care Coordination Team by providing clinical and administrative supervision, conducting performance reviews, and coaching staff to improve field and office skills. Manage staff recruitment, hiring, and development through individualized training plans and workshops while tracking work activities including outreach, intake, assessment, and service delivery. Ensure Medicaid compliance by monitoring billing processes, conducting quality assurance reviews, and verifying member chart documentation against Health Home agreements. Respond to inpatient and ER admission alerts, serve as the point of contact for crisis intervention, and maintain 24/7 availability for emergencies. Utilize Electronic Health Record systems to maintain client assignments and submit monthly billing reports by the first business day of each month.
Required Qualifications
- Masters degree
- 4 years of experience working with the seriously mentally ill to supervise the provision of services to Medicaid recipients with chronic medical and mental health issues
- Must be knowledgeable about Health Home regulations
- Must be comfortable working with clients in the field
