Program Specialist, Philadelphia County

Coordinate referrals, intake, assessments, enrollment, and service planning for individuals with intellectual and developmental disabilities. Monitor treatment interventions and outcomes, coordinate services and activities, and maintain documentation while reviewing records for compliance. Serve as a liaison with families, service coordinators, and community resources. Provide staff training, supervision, and support related to service plan implementation to ensure quality services and adherence to agency standards. Requires a BSW with at least 2 years of direct experience supporting individuals with I/DD, or an MSW with 1 year of direct experience. Knowledge of HCSIS, HRST, and EHR systems is preferred. This temporary, full-time position spans 2-3 months within Philadelphia County.
