Job Summary
Coordinate treatment for a caseload of individuals in a residential program by working collaboratively with internal and external stakeholders to implement Individual Support Plans. Ensure all identified supports and services are provided, relevant, and compliant with state and federal funding requirements. Act as a liaison between departments to coordinate annual reports, obtain accurate documentation, and maintain benefits without lapse. This role offers a flexible daytime schedule and is based in an office in Berwyn, PA. Devereux Adult Services is a leading nonprofit dedicated to unlocking human potential for individuals with emotional, behavioral, or cognitive differences.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, or related field
- Two (2) years' experience in the Human Services field, working with adults with intellectual/developmental disabilities, mental health diagnoses, and/or TBI
- Knowledge of regulatory requirements and an ability to adhere to established regulations
- Excellent customer service skills
- Written and verbal communication skills
- Organizational skills
- Problem-solving skills
- Ability to adapt to rapidly changing demands of the position, customer needs, and the organization
- Proficiency in Word Processing and Database management
- Valid driver's license
- At least 21 years of age
