Job Summary

Coordinate client services from referral through ongoing delivery, managing admissions, onboarding, and caregiver matching based on client needs and schedule requirements. Maintain regular communication with clients, caregivers, families, and referral partners while monitoring schedules, service changes, and continuity of care needs. Track authorizations, documentation, and follow-up items in agency software, responding to concerns in a timely, solution-focused manner. Assist with case management activities, quality assurance, and staff orientation as requested. Escalate unresolved service or compliance issues to leadership. Participate in case reviews, meetings, and training programs. Operate primarily in an office or remote environment with minimal travel.

Required Qualifications

  • High school diploma or equivalent
  • Associate degree or additional education in healthcare, human services, business, or related field
  • One (1) to two (2) years of experience in home care, healthcare, human services, customer service, scheduling, staffing, or care coordination
  • Strong organizational skills
