Job Summary
Act as liaison between provider network and nephrology practices; facilitate transition to Value-Based Care; serve as primary contact for IWH Network practices; communicate complex value-based care objectives to internal/external stakeholders; provide customer service and issue resolution to providers and internal teams; execute company initiatives and ensure timely deliverables; deliver training/onboarding to Network Providers; verify accuracy of provider information in CRM; travel up to 50% to practices; collaborate with external partners and cross-functional teams.
Required Qualifications
- Bachelor’s degree required; Advanced Degree desirable
