Job Summary

Coordinate pharmacotherapy needs for patients with Non-Tuberculous Mycobacteria, Bronchiectasis, and Cystic Fibrosis in the outpatient practice, maintaining contact with assigned caseloads and initiating responses to clinical issues. Develop and implement clinical practice guidelines, optimize workflow policies, and provide patient and family education to ensure medication safety and satisfaction. Partner with physicians, nurses, and social workers to execute specialized protocols, monitor clinical indicators, and document adverse drug reactions. Lead program initiatives, quality assessment activities, and investigator-initiated trials while advancing research productivity. Requires PGY1 and PGY2 pharmacy residency training, active New York State licensure, and Board certification in Pharmacotherapy within 18 months. Part-time schedule (17.5 hours) with salary range of $84,577.93 - $131,407.13.

Required Qualifications

  • Doctor of Pharmacy degree from an ACPE accredited school of pharmacy
  • Active New York State Pharmacist Licensure
  • PGY1 pharmacy residency training
  • PGY2 pharmacy residency training or equivalent salaried professorship at a school of pharmacy
  • Board certification in Pharmacotherapy or other specialty
  • At least four years of pharmacy experience
  • Effective leadership skills
  • Self-directed
  • Able to work independently
  • Excellent interpersonal, communication and organizational skills
  • Creative problem solving and critical thinking ability
  • Understanding of complex clinical care protocols
  • Familiarity with state and federal regulations related to medications
  • Qualified candidates must be able to effectively communicate with all levels of the organization
