Job Summary
Program Manager for Long Term and Community Integration Services (LTCIS) responsible for supervising and training assigned staff, developing and implementing Individual Service Plans (ISP), ensuring regulatory compliance (Medicaid, Licensure, CARF) and adherence to company policies. Coordinates services with participants, families, and community agencies; maintains accurate documentation and reports (evaluation/assessments, annual plans, progress notes, incident reports, and case notes). Leads PCP planning, ensures staff conduct, maintains participant files, and upholds safety and quality standards. Requires participation in ODD competencies, knowledge of behavior intervention techniques, and collaboration with support coordinators. Strong communication, problem-solving, and Microsoft Office fluency are essential; driving may be required depending on site. The role emphasizes supervision, staff development, regulatory compliance, and delivering meaningful services to individuals with disabilities.
Required Qualifications
- High school diploma or General Educational Development required
- Three (3) years’ experience in the human service field
- One year supervisory experience
- Must pass State background checks including fingerprinting (DBHDS)
- Must pass the Orientation to Developmental Disabilities (ODD) assessment and competencies
- Current valid driver’s license with acceptable MVR
- Ability to drive/participate in community transportation as required
- Proficiency with Microsoft Office; ability to learn corporate programs (e.g., Microsoft Teams, Zoom)
- Excellent verbal and written communication skills
- Detail-oriented with the ability to work independently and as part of a team
- Knowledge of behavior intervention principles and community resources
- Ability to coordinate with support coordinators and other agencies
- Maintain participant files in compliance with Medicaid, Licensing, and CARF standards
