Job Summary
Oversee assigned Community Living homes by coordinating healthcare appointments, leading Person-Centered Support Planning, and ensuring medical and community integration goals are met. Hire, train, and supervise direct care staff while managing schedules, payroll, and performance evaluations within budget. Maintain safe, clean environments by coordinating landlord maintenance and managing individual financial stability. Ensure timely completion of shift documentation, event reports, and Medicaid-compliant progress reports to support DMH and CARF accreditation standards. Collaborate with stakeholders including DFS, SSA, and internal teams to prevent service interruptions. Requires on-call coverage during the week and rotating weekend schedule.
Required Qualifications
- A high school diploma or GED
- Eligible to qualify as a Department of Mental Health (DMH) Degreed Professional Manager (DPM)
- Possess a Class E Missouri Driver's License
- Proof of vehicle insurance
- Ability to lift up to 50 pounds
- Ability to bend, stoop, and stand for significant periods
- On-call coverage during the week
- Rotating weekend schedule
- Direct care coverage availability in emergency staffing situations
