Job Summary
Program Facilitator leads and supports individuals with disabilities through community integration and skill development, supervising the Day/Community Team, developing program curriculum aligned with ISP goals, advocating and mentoring participants, coordinating staff, ensuring safety and compliance, maintaining participant files, and collaborating with families and state agencies. The role requires strong organizational, communication, and leadership abilities, plus the ability to work flexible hours and maintain documentation and case records.
Required Qualifications
- High School Diploma or GED required
- Experience working with people with disabilities required
