Job Summary
Program Coordinator responsible for overseeing and delivering direct client care at the assigned site, ensuring safety and well-being, training staff, and enforcing care plans. Responsibilities include providing ADL support and personal care, implementing person-centered programs, collecting data, preparing quarterly progress reports, participating in interdisciplinary team meetings, coordinating meals and household supplies, supervising site cleanliness, overseeing medication management and MAR documentation, training new staff, and ensuring licensing and compliance per 245D standards. Includes budgeting duties (Divvy transactions), collaboration with RN for medication management, and ongoing quality improvement and documentation tasks. Strong emphasis on teamwork, client-centered care, regulatory compliance, and effective communication within the BrightPath team.
Required Qualifications
- Desirable Qualifications: Experience working with individuals with a vulnerable population in a healthcare oversight capacity.
- Leadership experience and/or training in disability services.
- A minimum of 50 hours of education and training related to human services and disabilities.
- Must pass a Department of Human Services background check and maintain qualified status.
- Must have a current valid driver's license and insurance as required by state law.
- Proficiency in computer usage aligned with client healthcare needs and overall agency standards.
